Job summary

Eliot Community Human Services in Massachusetts seeks a Housing First Coordinator to support unsheltered adults in transition to housing, delivering person-centered, strengths-based case management.

You will coordinate needs assessments, action plans, and connections to community resources, advocate for client-centered services, and work onsite with flexibility. Compensation ranges from $48,000 to $50,000 per year, with generous benefits and professional growth opportunities.

Qualifications

• Bachelor’s degree in a human services field or social work or related field, preferred
• 2 years case management or comparable experience with the homeless population or lived experience with substance use disorder, homelessness, or mental illness

Responsibilities

• Work onsite at the office and in the field with flexibility to respond to needs as they arise throughout the day
• Engage in client-centered/harm-reduction/Housing First framework
• Forge and maintain positive relationships with community members and coalitions
• Advocate to ensure client interactions are person-centered/strength based/recovery oriented
• Collaboratively develop and implement needs assessments and action plans with clients
• Address ongoing clinical and/or systemic needs with attention to risk factors
• Develop and implement safety response plan as needed
• Knowledge of area resources and providers is a plus
• Familiarity with homelessness, substance use, mental illness and trauma is a must
