Job summary

The Worker in Phoenix seeks a Housing Expert who will work with staff, partners, and clients to deliver housing solutions and reduce homelessness. You will prescreen clients, provide coaching, and support them through move-in and stabilization, while tracking outcomes.

You will generate data-informed reports, coordinate wraparound services with Televerde Foundation and other partners, attend weekly meetings, document progress, and ensure data accuracy across systems.

Qualifications

• BS in Social Work or related field preferred.
• 4+ years experience in related field is preferred.
• Professional and/or volunteer experience with direct service preferred.

Responsibilities

• Review prescreens to determine program fit based on guidelines.
• Schedule intakes quickly to enroll clients.
• Walk clients through background check process and explain results.
• Assist clients with move-ins after program approval.
• Support clients at move-in and move-out using checklists.
• Provide one-on-one life skills coaching (savings, budgeting, housing prep).
• Meet with clients weekly to check progress.
• Update client notes in Salesforce within 24 hours of meetings.
• Collaborate with Televerde Foundation and partners for wraparound services.
• Attend weekly case-conferencing meetings for coordinated care.
• Share monthly resources with clients.
• Document exits promptly and accurately in client files.
