Job summary

Responsibilities

• Provide case management for veterans and their families using Housing First.
• Assess needs, develop housing stability plans and conduct home visits.
• Manage assigned caseload including HUD-VASH and related programs.
• Coordinate with housing resources, shelters, and VA/coalitions.
• Maintain accurate case files and comply with grant policies.
• Participate in staff meetings and professional development.
