Job summary

Mel Trotter Ministries in Grand Rapids is seeking a Housing Stabilization Case Manager to help neighbors experiencing homelessness maintain housing and pursue healthier, steadier lives. You will provide trauma-informed, individualized case management to strengthen stability and prevent returns to homelessness.

Your days include in-home visits, coordinating resources, coaching practical skills, and documenting progress, with ongoing collaboration with your program manager to stay responsive to

Qualifications

• Bachelor’s in social work/human services or relevant human services experience (preferred).
• Case management experience (preferred); lived experience with homelessness (preferred).
• Valid driver’s license and willingness to transport clients.
• Strong written/verbal communication, problem-solving, and adaptability.

Responsibilities

• Build client-centered plans focused on housing retention, wellness, and financial stability.
• Coordinate resources and services; connect clients to community supports.
• Use motivational interviewing to coach progress toward goals.
• Maintain timely, accurate documentation using databases and common office tools.
