Job summary

Bayaud Works is seeking a Case Manager to provide trauma-informed, housing-focused case management for families staying at the Tamarac Family Emergency Non-Congregate Shelter. You will assess barriers, develop stability plans, and link residents to wraparound services to promote long-term housing stability, income growth, and well-being.

Responsibilities include documentation, collaboration with partners, and adherence to Housing First standards.

Qualifications

• Knowledge of housing systems and family homelessness in the Denver Metro area.
• Strong interpersonal, communication, and problem-solving skills.
• Ability to provide trauma-informed, equitable, and culturally responsive services.
• Strong organizational skills with attention to detail in documentation.
• Ability to build relationships with residents, landlords, and service providers.

Responsibilities

• Provide housing-focused case management services, including intake, assessment, service planning, referrals, follow-up, and stabilization support.
• Assess resident strengths, housing barriers, service needs, income needs, benefits eligibility, and other factors affecting stability.
• Develop and update individualized housing stability and employment plans with clear goals and action steps.
• Support residents with housing navigation, documentation, landlord communication, benefits, employment resources, health care connections, and other supports.
• Maintain accurate documentation in HMIS and case files in alignment with program standards.
• Participate in case conferencing, team meetings, and coordination with shelter staff and partners.
