Job summary

Bayaud Works in Denver is seeking a Case Manager to provide trauma-informed, housing-focused case management to families at the Tamarac Family Emergency Non-Congregate Shelter. You will identify housing barriers, develop stability plans, and connect residents to wraparound services to promote long-term stability and self-sufficiency.

You will maintain documentation in HMIS and other systems, collaborate with shelter staff and community partners, advocate with landlords, and support crisis

Qualifications

• Associates degree in Social Work, Human Services, or related field preferred; equivalent experience considered.
• Minimum 2 years experience in case management, housing navigation, or human services.
• Experience with HMIS or similar data systems preferred.
• Must pass a background check.

Responsibilities

• Provide housing-focused case management services, including intake, assessment, service planning, referrals, follow-up, and stabilization support.
• Assess resident strengths, housing barriers, service needs, income needs, benefits eligibility, and other factors impacting stability.
• Develop and update individualized housing stability and employment plans with clear goals, action steps, timelines, and measurable progress.
• Maintain accurate and timely documentation in HMIS, case files, and other required systems.
• Advocate with landlords, housing providers, service agencies, schools, and other partners; support resident self-advocacy.
