Directions for Living is seeking a Housing Stability Case Manager for the Family Works program within the Housing First Division. The role involves providing case management, coordinating services across mental health, social, and educational domains, and partnering with families to secure stable housing and resources.

Qualifications

• Bachelor's or Master's in Social Work or related field from accredited university.
• Experience with child welfare, early childhood education or children's mental health preferred.
• Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word.

Responsibilities

• Provide case management in the Family Works program within Housing First Division.
• Coordinate and monitor services from mental health, social, and educational areas.
• Develop individualized treatment plans with IDT in collaboration with family.
• Deliver services in-home or in the community, focusing on housing stability.
• Ensure trauma-informed, culturally competent care and use of community resources.
