Job summary

Bayaud Works is seeking a full-time Case Manager for the Tamarac Family Shelter in Denver, Colorado. The role focuses on trauma-informed, housing-focused case management for families in non-congregate shelter settings.

You will identify housing barriers, develop stability plans, coordinate wraparound services, and connect families to resources to support long-term stability and self-sufficiency. The position requires strong documentation, collaboration with partners, and adherence to Housing

Qualifications

• Associates degree in Social Work or related field preferred.
• Minimum 2 years of experience in case management, housing navigation, or human services.
• Experience (1 year) with HMIS or similar data systems preferred.
• Experience (1 year) working with families experiencing homelessness preferred.

Responsibilities

• Provide housing-focused case management including intake, assessment, planning, referrals, follow-up, and stabilization support.
• Assess resident strengths, housing barriers, service needs, benefits eligibility, and other factors affecting stability.
• Develop and update individualized housing stability and employment plans with clear goals and timelines.
• Support residents with housing navigation, documentation, landlord communication, benefits, and health care connections.
• Use trauma-informed, culturally responsive approaches to promote dignity and resident engagement.
• Maintain accurate documentation in HMIS and program systems in alignment with standards.
• Participate in case conferencing, team meetings, and coordinated service planning.
