Qualifications

  • Bachelor’s degree in social work, human services, psychology, sociology, or a related field.
  • Two years of relevant experience preferred.
  • Knowledge of trauma-informed care and housing retention practices.

Responsibilities

  • Conduct comprehensive family assessments within 48 hours of admission and complete intake documentation.
  • Develop, implement, and monitor service plans and ILPs to promote housing stability.
  • Provide ongoing case management, crisis intervention, counseling, and trauma-informed support.
  • Advocate for clients and connect to benefits, healthcare, education, and community resources.
  • Assist families in understanding lease agreements and financial management to prevent arrears.
  • Monitor progress, follow up regularly, and document in CARES and other systems.
  • Collaborate with property managers and partners to support housing retention and well-being.
