Job summary

Intermountain Centers For Human Development, Inc. in Sierra Vista, AZ is seeking a Housing Specialist to provide direct housing support and 80% of case management in member homes and community locations.

You will work with members to develop individualized service plans, coordinate with behavioral health and medical teams, and advocate for stable housing and well-being in the community.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related field, or equivalent experience.
• Experience in the public behavioral health system and housing-related training or support preferred.

Responsibilities

• Provide Housing Based Case Management services to individuals with varying disability diagnoses.
• Complete comprehensive needs assessments to identify areas of global functioning and services.
• Develop an Individualized Service Plan and provide ongoing clinical services (Skills Training, Case Management, Vocational Services).
• Collaborate with member’s behavioral health provider, medical providers, law enforcement/probation, Child Services, landlords, family members and other stakeholders to support stable housing.
• Advocate for members with landlords and connect to adult programs/services promoting housing/income security and well-being.
• Provide supports to address needs for problem-solving, wellness, self-management, housing education and vocational training.
• Maintain accurate documentation of services via Electronic Health Record with daily productivity.
• May transport members in personal or company vehicles; provide 80% of treatment contacts in the community.
• May provide 24-hour crisis intervention on-call on rotating basis.
• Follow organization policies and perform other related duties as assigned.
