Job summary

WestCare California Inc is seeking a dedicated Case Manager to support participants enrolled in the Bridges to Success Adult Reentry program in Fresno. You will provide housing-focused case management and coordinate services to help clients achieve stable housing.

The role emphasizes working with formerly incarcerated individuals, coordinating with housing agencies, health services, and community resources.

Qualifications

• Minimum two years in social services, case management, or related field.
• Strong knowledge of housing programs and community resources.
• Excellent communication and interpersonal skills.
• Proficient with Microsoft Office; capable of data entry and reporting.
• CPR certification, First Aid certification, and TB test required.

Responsibilities

• Provide case management for a caseload of up to 25 participants.
• Navigate clients through Coordinated Entry and housing interventions.
• Deliver Rapid Rehousing services and promote housing stability.
• Maintain client files, service plans, and data in the Clinical Data System.
• Coordinate referrals to health, employment, and other services; report as required.
