Job summary

St. Joseph Center seeks a Case Manager I within the Office of Diversion and Re-entry to provide case management and support services for clients, including those exiting jail, across four Westside Los Angeles sites.

The role emphasizes coordinating housing, health, and benefits, while building strong community ties. You will work with a multidisciplinary team, maintain organized records, and ensure timely documentation and outreach.

Qualifications

• 1+ year of experience with unhoused individuals in social work/mental health
• Bachelor’s degree in Psychology, Social Work, Human Services or related field
• Ability to work flexible hours including some early mornings and evenings
• A bilingual background is a plus

Responsibilities

• Provide case management and supportive services to meet the needs of each individual.
• Build trust and rapport with individuals who are exiting the jail system.
• Develop and maintain strong ties to the court systems, community, and law enforcement.
• Link clients to appropriate bridge/interim housing programs and assist them in completing required paperwork to obtain appropriate permanent housing.
• Develop and maintain strong collaborative relationships with other service providers involved in the client’s care.
• Respond to referral assignments within program expectations and initiate/complete tasks prior to client release date.
