Job summary

HomeFirst in Santa Clara County seeks an Outreach Specialist to engage unsheltered individuals, connect them with TOEP services, and coordinate with Case Managers and Housing Specialists to re-engage participants.

You will perform intake, document in HMIS within 3 business days, and work with landlords and partners to find housing solutions. Some weekend/after-hour support may be required.

Qualifications

• High school diploma or GED; bachelor’s degree in social work or related field is a plus.
• Experience with homelessness and community partnerships.
• Strong written and oral communication; ability to build trust.
• Ability to manage multiple projects and meet deadlines.
• Understanding of Fair Housing law and tenants’ rights.
• Valid CA driver’s license and reliable transportation.
• Proficiency with MS Office and HMIS is preferred.

Responsibilities

• Direct outreach to homeless individuals in Santa Clara County.
• Link TOEP participants with Case Manager and Housing Specialist.
• Engage landlords to secure housing opportunities.
• Perform intake assessments to determine eligibility.
• Document case management activity in HMIS within 3 business days.
• Provide access to emergency housing as needed.
• Attend relevant meetings and maintain records.
• Weekend and after-hour support as needed.
• Collaborate with team and follow agency policies.
