Job summary

The People Concern is seeking a Temporary Case Manager for Kensington Campus in Lancaster, CA. You will support Clients’ housing stability and service coordination, maintaining a caseload and collaborating with local health departments.

This onsite role operates Mon-Fri, 8:00am-4:30pm, through November 2026 with possible extension. Responsibilities include intake, assessments, and goal planning, plus coordinating transportation and benefits to help Clients transition from homelessness to

Qualifications

• High School Diploma or GED or Equivalent.
• Bachelor’s degree preferred; or 2 years’ experience with homeless individuals.
• Valid CA Driver’s license and reliable vehicle.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients for intensive case management.
• Collaborate with DMH and DHS programs to coordinate treatment teams.
• Screen, enroll and orient Clients to program policies and goals.
• Complete biopsychosocial assessments to guide treatment planning.
• Develop quarterly goals to improve quality of life and health outcomes.
• Maintain accurate progress notes and HIPAA-compliant records.
• Coordinate appointments, transportation, and follow-up services for access to care.
• Transport Clients in personal vehicle to agencies and providers.
• Teach life skills such as budgeting, housekeeping, and scheduling.
• Work with property managers and cross-functional teams weekly.
