Job summary

Transition Projects, Inc. in Portland, OR is seeking an In-Reach Case Manager I to facilitate housing placements, promote housing stability, and support participant engagement with shelter partners.

The role centers on helping participants attain stable housing and move toward self-sufficiency through outreach, advocacy, and coordinated services. The In-Reach Case Manager I works Monday–Friday, 8:00am–4:00pm, with some weekend or evening work as needed, earning $29.00 per hour.

Qualifications

• Bachelor's Degree in a related field or 2 years of experience in social services.
• Demonstrated ability to interact with other agencies and advocate for participants.
• Knowledge of chemical dependency and mental health resources.
• Valid driver’s license and reliable transportation.

Responsibilities

• Facilitate development and implementation of individual goals to attain stability and housing.
• Address barriers to goals including mental health, substance use, income development, tenant education, socialization, basic needs.
• Identify, locate and coordinate delivery of services for participants.
• Meet with participants to provide and coordinate services.
• Coordinate with agency staff to ensure quality and comprehensive services.
• Develop and maintain relationships with community resources for participant access.
• Coordinate housing placement and retention services.
• Maintain participant files and HMIS documentation.
