Job summary

Transition Projects, Inc. is seeking a Temporary In-Reach Case Manager at our Macadam Office in Portland, OR. The role focuses on facilitating housing placements, promoting stability, and supporting participants toward self-sufficiency.

You will conduct home visits, coordinate services via HMIS, work with shelters, and provide engagement activities. The position requires strong organization, rapport-building, and ability to collaborate with community partners.

Qualifications

• Bachelor's degree or 2 years in social services.
• Ability to coordinate with housing/agency partners.
• Knowledge of chemical dependency and mental health resources.
• Bilingual English/Spanish preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate housing placement and retention services.
• Develop and implement individual goals to attain stability and housing.
• Meet with participants to provide and coordinate services.
• Maintain participant files and HMIS documentation.
• Attend staff and partner meetings; facilitate engagement groups.
