Father Joe's Villages in San Diego is seeking a full‑time Case Manager to support individuals experiencing homelessness. You will conduct intake assessments, build individualized plans, and coordinate housing resources, benefits, and services to help clients achieve housing stability and self‑sufficiency.

The role emphasizes compassionate, client‑centered support, collaboration with teams, and meticulous documentation in HMIS.

Qualifications

• Bachelor's degree or equivalent combination of education and experience.
• Minimum one year of experience with underserved or at‑risk populations in social services/housing.
• Knowledge of trauma‑informed care, motivational interviewing, strengths‑based engagement, and person‑centered approaches.
• Ability to assess client needs and coordinate services across multiple systems.
• Strong documentation and follow‑through abilities, including case notes and referrals.

Responsibilities

• Conduct intake interviews and comprehensive assessments to identify client needs and goals.
• Develop, implement, monitor, and update individualized service plans focusing on housing stability and income growth.
• Maintain an assigned caseload and provide ongoing case management support.
• Connect clients to housing resources, benefits, healthcare, behavioral health, employment programs, and community supports.
• Provide advocacy, crisis intervention, and service coordination as needed.
• Prepare timely case notes and maintain HMIS records and required documentation.
