Job summary

Valid California Driver’s License required.

Responsibilities

Provide housing stabilization and tenancy support to residents in Permanent Supportive Housing.
Maintain regular contact with residents and address housing-related concerns.
Help residents understand lease requirements, tenant responsibilities, and program expectations.
Connect residents with healthcare, behavioral health, benefits, and community resources.
Partner with landlords and property managers to support successful tenancies and address concerns.
Conduct housing visits and other community-based activities.
Maintain accurate and timely case notes, housing records, and required documentation.
Collaborate with internal teams and community partners to coordinate services and support residents.
