Job summary

Father Joe's Villages in San Diego is seeking a Case Manager I to support individuals, families, and TAY as they move into stable housing. You will manage a caseload, develop plans, and connect clients to rental assistance and services to promote long-term stability.

This full-time role requires a Bachelor’s degree in Social Work or related field and at least two years’ experience, with strong HIPAA knowledge, organizational and communication skills. A driver’s license is preferred for travel.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Two years of full-time experience in case management or related field preferred.
• Strong HIPAA knowledge and ability to maintain confidentiality.
• Proficiency with Microsoft Office and accurate data entry.

Responsibilities

• Manage a caseload of up to 23 households and provide direct services.
• Conduct regular meetings and monthly home visits to assess needs and progress.
• Develop and maintain solution-focused case plans for housing retention.
• Connect clients to rental assistance and community resources.
• Advocate for clients with landlords and service providers to resolve issues.
• Travel throughout San Diego County to support client engagement.
• Maintain timely documentation in CSTAR per HIPAA and program requirements.
• Follow program policies and documentation standards.
