Job summary

Coordinated Care Agency (CCA), a dba of the Archdiocesan Housing Authority, seeks a Case Manager to engage clients and provide harm-reduction oriented supportive services to help housing stability.

The role involves coordinating with providers, documenting progress, and meeting program guidelines to assist clients in obtaining or maintaining housing and following through on individualized stability plans.

Qualifications

• Minimum Qualifications: Associate’s degree in social work or related field or equivalent experience.
• One to two years’ experience in housing and/or employment assistance.
• Must attend required meetings and trainings.
• Understanding of challenges faced by homeless individuals and families, including substance use, trauma, and mental health issues.
• Proficiency with Microsoft Office; strong written and oral communication; ability to learn client services database.

Responsibilities

• Work with clients to identify barriers and document housing stability plans.
• Build rapport and advocate for diverse clients using harm reduction approaches.
• Participate in verbal de-escalation and intervene in emergent situations.
• Provide support and mediate disputes between clients.
• Adhere to program guidelines and regulations.
• Assist clients in obtaining/maintaining housing and accompany to external appointments as needed.
• Meet with clients regularly to monitor progress toward goals.
• Refer clients to community resources and maintain cooperative relationships with current services.
• Maintain accurate timesheets and document hours worked.
• Attend staff meetings and trainings.
• Work in-person at a program and participate in agency meetings.
