Job summary

The People Concern is seeking a Case Manager for its 1308 Harm Reduction program in Santa Monica. This onsite role supports housing stability, self-sufficiency, and community integration for clients facing homelessness through evidence-based practices including Housing First, harm reduction, and trauma-informed care.

You will manage a caseload, coordinate services, and assist with documentation, benefits linkage, and employment pathways to promote permanent housing outcomes.

Qualifications

• High School Diploma or GED or equivalent.
• Valid CA Driver's license, reliable car, auto insurance.
• Willing and able to transport Clients in your personal vehicle.
• Experience with mental illness, chronic health issues, substance abuse disorders.
• Experience with outreach/engagement, crisis intervention, housing-related work.
• Excellent communication and professional demeanor.
• Detail oriented with strong time management.
• Proficient with Microsoft Office suite.

Responsibilities

• Manage a caseload of approximately 22–25 clients with weekly 1:1 meetings.
• Transport clients in personal or company vehicle when needed.
• Lead housing-related case management and maintain active housing plans.
• Utilize community resources to provide wrap-around services for housing placement.
• Address barriers to housing with linked income/benefits and health services.
• Provide Housing First, harm reduction, and trauma-informed care approaches.
• Conduct initial and ongoing needs assessments for housing stability.
• Develop and monitor individualized housing and service plans.
• Maintain HMIS documentation and compliant participant files.
• Facilitate intakes for interim housing and coordinate care across teams.
• Attend required meetings and trainings; maintain releases of information.
