Job summary

SHELTER, Inc. in Pittsburg, CA is hiring a Case Manager for our Domestic Violence Rapid Rehousing Program. You will deliver compassionate wraparound services to individuals and families facing homelessness, with a caseload of about 15 households.

This role emphasizes housing stability, collaboration with community partners, and documentation in HMIS, with a focus on empowering clients to long-term self-sufficiency.

Qualifications

• 1+ years' experience working with low income and/or homeless populations in wraparound services.
• Proficient computer skills including Microsoft Office Suite (Outlook, Word and Excel).
• Experience working in diverse settings with people across all socio-economic spectrums.

Responsibilities

• Screen participants, verify eligibility, and assess service needs to determine housing barriers.
• Develop housing stability plans and connect participants to resources and support services.
• Provide wraparound services, financial coaching, and ongoing support for long-term stability.
• Coordinate referrals to SHELTER, Inc. programs and partners to ensure access to services.
• Maintain a caseload of about 15 households with regular check-ins and care coordination.
• Reassess participant needs and adjust service plans to support housing outcomes.
• Manage enrollments, transitions, and exits per agency policies and funding requirements.
• Document services and outcomes in HMIS and required systems.
