Job summary

SHELTER, Inc. in Pittsburg, CA is hiring a Case Manager for its Domestic Violence Rapid Rehousing Program.

You will conduct screenings, determine service needs, and create housing stability plans to help families escape homelessness and achieve lasting stability. Join a compassionate team providing wraparound services, financial coaching, and ongoing support while coordinating referrals and maintaining thorough documentation.

Qualifications

• 1+ years' experience working with low income and/or homeless populations in wraparound services.
• Proficient computer skills, including Microsoft Office Suite (Outlook, Word and Excel).
• Experience working in diverse settings with people across socio-economic spectrums and a wide variety of roles.

Responsibilities

• Conduct participant screenings, verify eligibility, and complete program assessments to determine service needs and housing barriers.
• Develop individualized housing stability plans with participants, setting achievable goals and connecting them to resources.
• Provide strengths-based case management, financial coaching, and ongoing support for long-term stability.
• Coordinate referrals to SHELTER, Inc. programs and community partners, advocating for participants and ensuring access to services.
• Maintain a caseload of approximately 15 households with regular check-ins and progress monitoring.
• Reassess participant needs and adjust service plans to support housing outcomes.
• Manage enrollments, transitions, and exits in line with agency policies and funding.
• Document services, case notes, and outcomes in HMIS and required systems.
