Job summary

Shelter House, Inc is seeking a Diversion First Case Manager to manage a caseload of 18-19 households in the Diversion First PSH program. The role focuses on housing stability through trauma-informed, strengths-based case management and close collaboration with the CSB and partner agencies.

The position requires a bachelor’s in human services or related field, 2+ years of experience, and a willingness to work evenings and weekends when needed. A valid driver’s license and vehicle are required.

Qualifications

• Bachelor's degree in human services or related field or commensurate experience.
• 2+ years of case management/direct services experience.
• Strong written and oral communication skills.

Responsibilities

• Conduct initial, person-centered assessments to identify client strengths and goals.
• Develop housing service plans with short- and long-term goals.
• Coordinate referrals to housing, healthcare, employment, and benefits.
