Job summary

Adjoin seeks a dedicated Disability Services Specialist to manage a mixed caseload, provide housing navigation, assess needs, and coordinate housing resources with emphasis on empowerment and community integration.

You will coordinate tenant education and tenancy supports, ensure compliant documentation, and collaborate with agencies, landlords, and families to promote stable living arrangements for clients with disabilities. Travel regionally will be required.

Qualifications

• 18 years of age or older.
• ASL and/or LSM knowledge preferred to strengthen accessibility.
• Bachelor's degree or three years equivalent experience with individuals with disabilities.
• English and Spanish proficiency required where feasible.
• Knowledge of Title 17 Housing Access Services and affordable housing resources desirable.
• Strong interpersonal, organizational and documentation skills.
• Ability to travel regionally and statewide as required.

Responsibilities

• Manage a mixed caseload including Housing Access Services participants and other Catalyst programs.
• Process referrals from appropriate agencies and expedite intake and assessment.
• Provide housing navigation services and coordinate documentation for housing stability plans.
• Coordinate affordable housing resources and assist participants with eligibility and applications.
• Educate tenants on rights and responsibilities and assist with reasonable accommodations.
• Support eviction-prevention and tenancy stabilization efforts.
• Lead and develop direct support staff where applicable.
• Maintain accurate documentation and logs; review staff documentation for quality.
