Job summary

Adjoin seeks a dedicated Disability Services Case Manager for our Orange, CA location. This full-time role focuses on housing navigation, client assessments, and coordinated supports to help individuals achieve stable living situations.

You will manage a mixed caseload, collaborate with housing authorities and community partners, and document activities in our case management system. Bilingual ability and travel are required or expected.

Qualifications

• 18 years of age or older.
• ASL and/or LSM knowledge preferred for accessibility.
• Bachelor's degree or 3 years equivalent experience with disabilities.
• Experience as Catalyst Case Manager or comparable role preferred.
• English and Spanish proficiency where feasible.
• Knowledge of Title 17 Housing Access Services or housing resources preferred.
• Ability to multitask, follow detailed instructions and maintain confidentiality.
• Must have a valid CA driver's license and insured vehicle.

Responsibilities

• Manage a mixed caseload including Housing Access Services participants and Catalyst program participants.
• Process referrals and expedite the intake and assessment process.
• Provide housing navigation, including assessments, documentation, and stability plans.
• Coordinate with landlords, housing authorities, and community partners to support clients.
• Develop placements in work/living situations matching client strengths and needs.
• Supervise and develop direct support staff; review documentation for accuracy.
