Job summary

Aspire Community Services is seeking a Housing Case Manager to support adults with Intellectual and Developmental Disabilities in obtaining and maintaining safe, stable housing. You will apply a Whole Person-Centered Care approach to empower independence and community inclusion.

Responsibilities include assessments, housing plans, coordinating applications and benefits, and working with Supports Coordinators and providers to advocate for tenants and ensure housing stability.

Qualifications

• Must be 18 years or older.
• Valid driver’s license and reliable transportation.
• Ability to travel throughout the community for participant visits.
• Background clearances required.
• Experience with IDD or human services preferred.

Responsibilities

• Conduct comprehensive housing needs assessments and set goals.
• Develop individualized housing service plans using Whole Person-Centered Care.
• Assist with locating and maintaining safe, affordable housing.
• Coordinate housing applications, rental assistance and benefits.
• Collaborate with Supports Coordinators and providers.
• Advocate with landlords and housing authorities for participants.
