Job summary

Adjoin is seeking a dedicated Case Manager to oversee housing access and stability for clients with disabilities across California. You will coordinate housing resources, assess needs, develop stability plans, and collaborate with housing authorities, landlords, and service partners to ensure client-centered support.

Qualifications include a bachelor’s degree or equivalent experience, bilingual English/Spanish preferred, and strong interpersonal skills.

Qualifications

• 18 years of age or older.
• ASL and/or LSM knowledge preferred when feasible.
• Bachelor's degree or three years equivalent experience with individuals with disabilities.
• Experience as a Catalyst Case Manager or similar case management role preferred.
• English and Spanish proficiency where feasible.
• Knowledge of Title 17 Housing Access Services requirements.
• Strong interpersonal and communication skills.
• Computer literacy (MS Office) and record-keeping ability.
• Ability to follow multi-step instructions and manage time.
• First Aid and CPR certification or willingness to obtain within 45 days.
• Driver’s license and insured vehicle in good condition.

Responsibilities

• Manage a mixed caseload including Housing Access Services participants and other Catalyst programs.
• Process referrals and expedite intake and assessments.
• Provide housing navigation services and develop individualized housing stability plans.
• Coordinate affordable housing resources and assist with eligibility and documentation.
• Educate on tenancy rights and reasonable accommodations.
• Support eviction-prevention and tenancy stabilization efforts.
• Mentor and evaluate direct support staff and ensure documentation is accurate.
• Collaborate with landlords, housing authorities, and partners to coordinate access and supports.
• Maintain knowledge of program standards and Title 17 requirements.
• Participate in staff hiring, training, and evaluations.
• Complete logs, reports, and client evaluations in a timely manner.
