Job summary

CRCD is seeking an ICMS Case Manager to provide coordinated case management and supportive services for formerly homeless individuals with special needs. Services are on-site at a new permanent supportive housing complex, linking tenants to life skills, health services, counseling, and job training.

The role requires 1–2 years experience or a BA, strong organization, and the ability to work with diverse partners to sustain tenancy and independence.

Qualifications

• 1–2 years of experience or BA Degree.
• Experience with youth homelessness and housing instability.
• Reliable, flexible team player with good boundaries.
• Reliable transportation and valid driver’s license.
• Proficient in Microsoft Office.

Responsibilities

• Conduct initial housing intake and needs assessment with tenants.
• Develop Individual Service Plans (ISP) and link to services.
• Coordinate linkage to health, education, counseling and job training.
• Maintain monthly face-to-face contacts and quarterly progress reviews.
• Communicate with property management and mental health providers to safeguard tenancy.
• Ensure compliance with documentation, reporting, and program records.
