Job summary

Bond Health in the Bronx seeks a full-time Case Manager to provide residential and long-term support to homeless and at-risk adults transitioning into permanent housing. You will manage a caseload of about 20–25 clients, with regular in-person and outreach contacts.

This role requires a Bachelor's degree in human services, fluency in Spanish, strong writing and communication skills, and a proactive, community-focused approach. Prior experience with low-income or homeless populations is preferred.

Qualifications

• Strong organizational, writing, communication, and computer skills.

Responsibilities

• Provide direct case management on-site and in the field, including assessments and service planning.
• Develop and update Individual Service Plans and document progress toward goals.
• Coordinate with community providers and social services.
• Facilitate life skills, education, relapse prevention, and mental health counseling groups.
• Maintain electronic health records and tenant files with accuracy.
• Assist tenants with public benefits, Social Security entitlements, and housing-related needs.
• Collaborate with Workforce Opportunities staff to set employment goals.
• Ensure compliance with organizational and funder requirements.
• Conduct outreach, tenant engagement, and post-discharge follow-ups.
