Job summary

Housing Forward is seeking a Housing Case Manager to coordinate and deliver quality direct case management and stabilization services for participants in the Supportive Housing Program. The role includes housing advocacy and linkage services to support retention in permanent housing.

Responsibilities include developing service plans, coordinating with providers, documenting records, and providing life-skills training to help participants achieve greater independence and housing stability.

Qualifications

• Bachelor’s degree in social service preferred; Master’s degree considered or combination of education and experience
• Bi-lingual preferred
• Must have transportation capability (car, valid license, insurance)
• Smartphone to utilize agency tools

Responsibilities

• Provide case management services for program participants and coordinate care
• Work collaboratively to help clients obtain permanent housing and address barriers
• Deliver advocacy, crisis intervention, and counseling as part of case management
• Develop and monitor service plans in line with agency and funding requirements
• Complete internal/external reports and maintain accurate client files and logs
• Develop, coordinate, and facilitate groups and workshops as needed
• Maintain service linkages with local providers to support clients and families
• Transport clients to housing, legal, and medical appointments as needed
• Participate in clinical case conferences and care coordination
• Coordinate with the After Care Case Manager for follow-up up to 12 months post discharge
• Provide life-skills training to improve independence and quality of life
• Connect participants to medical, mental health, and other supportive services
• Maintain knowledge of community resources and referrals
• Conduct home visits to support goal work and lease compliance
• Assist with access to entitlements and eligibility
• Participate in trainings and team meetings
• Other duties as assigned
