Job summary

Volunteers of America Ohio & Indiana is seeking a Case Manager to develop service plans that promote stable, permanent housing and monitor clients for up to six months.

The role includes advocating for clients and coordinating with community providers to ensure access to resources that meet individual needs.

Qualifications

• Bachelor’s degree in human and/or social services or behavioral sciences.
• Education and/or verifiable work experience with at-risk populations is required (homeless, veterans, behavioral health, drug/alcohol addiction, employment, youth).

Responsibilities

• The Case Manager will be responsible for developing service plans designed to ensure stable, permanent housing and monitoring of our clients for up to six months.
• Incumbent acts as an advocate and liaison between the client and community service providers to ensure appropriate utilization of resources in meeting the needs of the client.

Education
