Job summary

Five Keys is seeking a Supportive Services Case Coordinator for its Rehousing Programs in San Francisco. You will provide housing-focused case management, helping participants secure housing and develop stability plans.

The role emphasizes a trauma-informed, person-centered approach and collaboration with landlords, agencies, and service partners. The position requires a strong commitment to housing stability, data integrity, and excellent written communication.

Qualifications

• Minimum 2 years of care coordination/case management with homeless or young adult populations preferred.
• Bachelor's degree preferred in social services fields.
• Strong written communication and organizational skills required.

Responsibilities

• Assess participant needs, strengths and challenges to plan housing stabilization.
• Assist participants in locating and securing housing and completing applications.
• Educate tenants on rights, lease requirements, and landlord communication strategies.
• Coordinate with rental assistance providers and community partners to promote housing stability.
• Enter data accurately in Salesforce, ONE System, and relevant city systems; prepare required reports.
