• Maintain confidentiality in all matters pertaining to participants and families.
• Develop trusting professional relationships to facilitate service delivery.
• Manage a caseload of 15–25 participants with weekly housing plan reviews.
• Assist with intake, assessments, and service planning per program guidelines.
• Ensure HIPAA compliance and accurate HMIS data entry.

Responsibilities

• Maintain confidentiality in all matters pertaining to participants and their families.
• Develop effective and trusting professional relationships with participants to facilitate provision of services.
• Maintain a caseload of 15-25 participants. Meet participants to develop housing plans based on the housing and service plan.
• Complete intakes and provide orientation to new participants and family members.
• Adhere to documentation standards, HIPAA policies, and HMIS data entry requirements.
• Maintain organized client files and submit HMIS case notes within 24 hours.
• Coordinate life skills curriculum and resources for participants.
• Respond to concerns and ensure timely follow-up on issues.
• Utilize evidence-based practices in service delivery to stabilize housing.
