Job summary

Bridgeways seeks a dedicated housing services coordinator to support Directors in managing the housing program. You will assist with housing placement, documentation, data entry into HMIS, and coordination with social service agencies to secure benefits and services for participants.

The role emphasizes direct client service, adherence to HIPAA, and collaboration with landlords and community partners to facilitate long-term tenancy and stability.

Qualifications

• Two years’ experience in social services or behavioral health.
• Experience with adults with chronic and persistent mental illness.
• Familiarity with local/state/federal low-income housing resources and homeless programs.

Responsibilities

• Ensure safekeeping of client information per HIPAA policies.
• Maintain attendance and adhere to scheduling requirements.
• Provide housing placement support and case management to facilitate access to safe, affordable housing.
• Document and submit paperwork accurately for services provided.
• Represent Bridgeways in community activities and liaise with housing agencies.
• Stay informed on Shelter Plus Care requirements and apply pertinent guidelines.
