Job summary

Community Bridges, Inc. (CBI) seeks a Client Care Coordinator to manage a housing caseload at shelter sites, coordinating care to help individuals and families ending homelessness. You will ensure connections to PCMH/ PCP services and coordinate with community providers to address social determinants of health.

The role requires documenting care activities, supporting leadership with reports, and working within a multidisciplinary team to achieve positive housing and health outcomes.

Qualifications

• Highschool diploma or GED required; associate degree preferred in Behavioral Health.
• Minimum of 12 months recovery from substance use and/or mental health disorders.
• 6+ months paid related work experience in behavioral/medical crisis exposure.
• 1-3 years full-time health care experience preferred; strong coordination skills.

Responsibilities

• Coordinate care and support services to help individuals end homelessness.
• Monitor RBHA and non-RBHA services and connect clients to housing options.
• Document interactions in data systems and prepare end-of-shift reports.
• Collaborate with a multi-disciplinary team including nurses, counselors, and clinicians.
