Qualifications

• Two years of experience in healthcare, case management, or with vulnerable populations is preferred.
• All levels of education and experience are welcome.

Responsibilities

• Conduct intake and develop Individualized Service Plans for participants.
• Provide referrals and support to help participants achieve their goals.
• Evaluate strengths and challenges through regular reviews.
• Maintain documentation standards as per program policies.
