Job summary

Father Bill’s & MainSpring is seeking a Re-Housing Case Manager in Massachusetts to locate and secure housing for guests in emergency shelter and outreach programs. The role prioritizes diverting from shelter and rapid re-housing, working within a team to assess and plan care.

Responsibilities include intake, service plan development, landlord outreach, transportation to viewings, and data maintenance. CPR is required; typical hours are Monday–Friday, 10am–6pm.

Qualifications

• 2–4 years of relevant experience in social services.
• Bachelor’s Degree in social work or related field (required).
• CPR certification is required and provided by FBMS.

Responsibilities

• Advocate for guests to move forward from homelessness and access housing options.
• Assess and develop exit plans and diversion strategies for shelter guests.
• Complete intake and assess strengths, needs, barriers, and goals.
• Coordinate with landlords, housing authorities, and service partners.
• Transport guests to viewings and appointments; pursue Rapid Re-Housing options.
• Maintain case records and submit service outcome reports as needed.
• Collaborate with housing team to ensure smooth transitions to permanent housing.
