Responsibilities

  • Complete home visits and assist participants in accessing resources and scheduling appointments.
  • Support applications for benefits and income resources (e.g., SNAP, MassHealth).
  • Refer and coordinate access to health, behavioral health, and legal services as needed.
  • Provide tenancy readiness coaching (budgeting, lease compliance, communication with landlords).
  • Assist with landlord/household mediation and problem-solving for tenancy-related issues.
  • Connect clients to housing retention and financial stabilization supports.
  • Provide structured follow-up and goal setting to ensure long-term tenancy sustainability.

Position Summary:

The Rapid Rehousing Stabilization Case Manager (RRSCM) provides immediate engagement upon referral from HRC exits and street outreach to ensure a warm handoff into stabilization services. The person in this position will work with clients to develop person-centered stabilization plans focused on housing retention, safety, income and benefit stability, tenancy skills, and preventing a return to homelessness. This role requires strong coordination with internal teams and external providers, including HRC staff, street outreach teams, Rapid Re-Housing providers, the front door diversion team, landlords, household members, and other community partners to support smooth transitions and continuity of care.

This position requires 2 years of relevant experience and/or a Bachelor’s Degree in social work or related field. Typical schedule is Monday-Friday 10a-6p, with some variation as necessary and may include some weekend days. CPR certification is required and provided by FBMS. AED and First Aid may also be required and will be provided by FBMS.

  • Deliver short-term, light– to medium-intensity stabilization services toindividualsexiting homelessness into permanent housingwhodo not have ongoing case managementsupports.
  • Screen for risk factors that drive housing loss (i.e., arrears, conflict with household members, lease compliance issues, safety concerns, lack of income/benefits, unmet health needs).
  • Identifyearly warning signs of destabilization and implement rapid problem-solving to address issues before they escape.
  • Complete home visits andassistparticipants in accessing community resources,scheduling,and attending appointments (transportation may berequired). Maintainaccuratecase records and collect information ina timelyand organized manner.
  • Support applications and follow-up for benefits and income resources (e.g., SNAP, SSI/SSDI supports, MassHealth/insurance navigation, employment resources, etc.).
  • Refer and coordinate access to health, behavioral health, substance use, legal, domestic violence, and other supportive services as needed.
  • Provide coaching around tenancy readiness and stability (e.g., budgeting, lease compliance, communication with landlords/household members).
  • Assistwith landlord/household mediation and problem-solving for issues such as guest policies and household agreements,noise/behavior complaints, building rule compliance, and repairrequests.
  • Connect clients to housing retention and financial stabilization supports, prioritizing existing resources before flex funding (e.g., RAFT, utilityassistance, food programs).
  • Provide structured follow-up (e.g., initial intensive engagement, then tapering contact as stability improves)and goal setting to ensure long-term tenancy sustainability.
  • Empathy: Anunderstanding andempathyfor those dealing with homelessness and related issues.
  • Professionalism:Maintainsthe highest level of professionalism in the delivery of services,building ofrelationships, collaboration with co-workers, and representation ofFather Bill’s &MainSpringwithin the organization and the community.
  • Communication : Speaks appropriately in positive and/ or negative conversations with supervisors, co-workers, volunteers, donors and constituents; Articulates the goals of Father Bill’s & MainSpring .
