Job summary

BHcare is seeking a Housing Case Manager to support individuals applying for and receiving housing subsidies in North Branford, CT. The role focuses on helping people secure and maintain permanent housing while connecting them to services and supports for long-term stability.

The position is full-time (37.5 hours/week, 8:30 AM–4:30 PM, Monday–Friday) with occasional evenings, weekends, and holidays to meet participant needs.

Qualifications

• Bachelor's degree required; 2+ years in human services preferred.
• Experience with homelessness, housing instability, or related challenges.
• Strong time management, organization, and prioritization skills.
• Ability to engage diverse populations respectfully and effectively.
• Proficiency with Microsoft 365 and record-keeping systems.
• Valid driver's license and insurable for agency business.

Responsibilities

• Assist with applications and eligibility for housing programs (HUD, DMHAS, DOH).
• Help clients obtain documentation and verification for housing assistance.
• Determine basic eligibility per program policies and CAN guidelines.
• Assist with housing search, placement, and lease retention strategies.
• Negotiate with landlords and advocate for participants to secure housing.
• Maintain regular contact via home visits, case coordination, and calls.
• Support transition from homelessness to permanent housing and lease compliance.
• Complete housing inspections, recertifications, and required documentation.
• Develop person-centered service plans with measurable goals.
• Provide outreach, crisis intervention, and linkage to resources.
• Assist in pursuing employment, education, and community involvement.
