Job summary

Community Bridges is seeking a Housing Specialist to work within a multidisciplinary team to implement permanent supportive housing for clients in Arizona. You will collaborate with housing agencies and landlords to secure safe, suitable placements and provide field support to clients.

The role requires driving and coordinating through the housing process, with emphasis on client safety and appropriate placement.

Qualifications

• High school diploma or GED required; associates degree or higher preferred.
• 1–3 years of behavioral health experience across settings.
• Minimum six months recovery from addiction or mental health conditions preferred.
• AZ Driver's License in good standing and clean MVR.
• Fingerprint clearance required; SOAR certification preferred; VI-SPDAT preferred.

Responsibilities

Housing Specialist

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Job Summary

The F/ACT Housing Specialist works with a multidisciplinary team to implement the permanent supportive housing model with members assigned to their respective F/ACT team. The F/ACT Housing Specialist is expected to work with community supportive housing agencies on temporary and permanent housing opportunities for their clients. The F/ACT Housing Specialist will provide in field support to clients and work with landlords to ensure ongoing safety to the client as well as the appropriateness of the placement

Skills/Requirements:

• Highschool Diploma or GED required. Associates Degree or higher in a field related to behavioral health preferred.
• 1-3 years of experience in behavioral health working in multiple settings and levels of care, including both mental health and substance abuse preferred.
• Minimum of six months of recovery from substance use and/or mental health disorders preferred.
• Current AZ Driver’s License in good standing.
• Clear 39-month Motor Vehicle Record.
• Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
• Fingerprint Clearance Card required.
• SOAR certification preferred.
• Certified to complete the Vulnerability Index Service Prioritization Decision Assistance Tool (VI-SPDAT) preferred.
