Job summary

Mission Action Access Point in San Francisco is seeking a Problem Solving Specialist to provide one-on-one support to individuals experiencing homelessness. You will work closely with the Licensed Care Coordinator and Housing Navigators to deliver coordinated assessment and housing navigation services, primarily from the Mission Access Point.

Qualifications

• At least 2 years of direct service experience in case management, counseling, or a similar field.
• Experience with homelessness.
• Bilingual in Spanish preferred.
• Strong written and verbal communication skills.

Responsibilities

• Screen and assess single adults experiencing homelessness using standardized tools and referrals.
• Provide client-centered, trauma-informed support to help exit homelessness.
• Collaborate with Housing Navigators and Care coordinators for coordinated assessment.
• Enter data in the ONE System and maintain accurate records.
• Provide professional, accessible, and culturally relevant support and advocacy.
