Job summary

Mission Action in San Francisco's Mission District is seeking a Problem Solving Specialist for the Access Point program. You will provide one-on-one support to individuals experiencing homelessness, coordinating assessments and referrals to housing options with the Housing Navigators.

Under the supervision of the Clinical Care Coordinator, you will use trauma-informed, client-centered approaches, document case notes in the ONE System, and contribute to harm-reduction and wellness within a

Qualifications

• At least 2 years of direct service experience in case management, counseling, or a similar field.
• Experience working with individuals experiencing homelessness is preferred.
• Strong written and verbal communication skills; trauma-informed approach.

Responsibilities

• Screen and assess single adults experiencing homelessness using standardized tools and referrals.
• Provide client-centered, trauma-informed support to help avoid shelter entry and street homelessness.
• Coordinate referrals into housing programs using the ONE System.
• Maintain accurate case records and follow up on outcomes in the ONE System.
• Collaborate with Housing Navigators to deliver coordinated services.
• Engage in staff training and team meetings; perform other duties as assigned.
