Job summary

Evergreen Health is seeking a Housing Retention Counselor to assist Erie and Niagara County residents in transitioning from homelessness to stable housing. You will assess needs, provide guidance, and connect clients with essential services to promote independence.

The role requires a Bachelor’s or Associate degree in health, human or education services with relevant experience, a valid NYS driver's license, and a dependable car for client activities. Equal opportunity employer.

Qualifications

• Bachelor’s or Associate degree in health, human or education services with qualifying experience.
• Qualifying experience includes care coordination for homelessness, chronic illness or mental health history.
• Experience with families preferred; extensive qualifying experience may substitute for degree.

Responsibilities

• Assesses independent living skills with care team to create service plans.
• Provides short-term situational counseling for patients in crisis.
• Helps patients secure services to maintain housing tenure.
• Participates in scheduled virtual and in-person meetings.
• Delivers customer service through a Trauma-Informed Care lens.
