Qualifications

• Valid driver’s license and ability to reliably travel throughout Erie County.
• Satisfactory completion of all required background screenings.
• Respect the confidential nature of all CCSI and CCSI customer information.

Responsibilities

• Process housing referrals submitted to the Adult Single Point of Access.
• Assist members of the community to navigate the mental health housing system.
• Facilitate meetings coordinating the mental health housing system of care.
• Assist with site monitoring visits of supportive housing grantees.
• Gather, organize and analyze data to inform quality processes.
