Job summary

Evergreen Health is seeking a Housing Retention Counselor to assist individuals across Erie and Niagara Counties in transitioning from homelessness to stable, permanent housing. You will guide clients, coordinate resources, and support stability while collaborating with a patient care team.

The role emphasizes trauma-informed care, strong communication, and the ability to work in a fast-paced environment with travel as needed. This is a full-time position with competitive benefits.

Qualifications

• Bachelor’s degree in health, human or education services or related field.
• Familiarity with Trauma-Informed Care (TIC) model.
• Valid NYS driver’s license and insured vehicle; ability to transport clients.

Responsibilities

• Assess independent living skills and work with care team to develop service plans.
• Provide short-term situational counseling for clients in crisis.
• Assist clients in securing services to maximize housing tenure.
• Participate in scheduled virtual and in-person meetings.
• Provide quality customer service through a TIC lens.
