MHMR of Tarrant County is seeking a Housing Specialist to work with clients on recovery management and housing stability. The role emphasizes a home visitation approach and collaboration with families, counselors, and team members to set 12-month goals and coordinate services through a local provider network.

Responsibilities include developing recovery plans, ensuring timely paperwork, and providing intensive case coordination for at least 20 clients.

Formación

• Minimum education: High School Diploma or GED.
• At least six months experience in the substance field.
• Valid Texas driver’s license with acceptable driving record.
• Substitutions: 6 months substance abuse life experience or a bachelor’s degree in health & human service.

Responsabilidades

• Assist in developing recovery management plans with clients, families, counselors and team members.
• Provide intensive case coordination to a minimum of 20 clients through routine utilization of a provider network.
• Maintain connectedness to each client’s growth and development by evaluating progression.
• Ensure clients receive services from the identified network of providers.
• Complete training requirements as specified by MHMR of Tarrant County Training Center.
