Job summary

StarCare Specialty Health System seeks a full-time MH Rehab/Supportive Housing & Employment Specialist to help adults with serious mental illness who are homeless or at risk of homelessness. You will provide housing navigation, rehabilitation, employment support, and skills training in a community setting.

Responsibilities include assessments, service planning, documentation, and coordination with providers to promote recovery, housing stability, and community integration.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or related field.
• Experience working with individuals with mental health conditions in supportive housing or related programs preferred.
• Knowledge of recovery-oriented practices and community resources preferred.
• Strong interpersonal, written, and verbal communication, organizational and documentation skills.
• Ability to work independently and with multidisciplinary teams.
• Proficiency in electronic health record systems and standard computer applications.
• Valid Texas driver's license and insurability.
• Ability to pass pre-employment screenings as required.

Responsibilities

• Provide community-based rehabilitation, housing stabilization, employment support, independent living skills training, psychoeducation, and resource coordination.
• Conduct participant assessments and reassessments; develop individualized service plans and progress notes.
• Assist participants in securing, maintaining, and transitioning to housing via navigation and coordination.
• Support budgeting, financial management, employment readiness, tenant responsibilities, and independent living.
• Coordinate with behavioral health providers, housing agencies, landlords, employers, benefits programs, and other partners.
• Maintain accurate documentation and electronic health records in accordance with policies.
• Foster effective communication within the team and with stakeholders.
• Participate in interdisciplinary meetings and quality improvement activities.
• Deliver person-centered, recovery-oriented services to promote independence and community integration.
