Qualifications

• Bachelor's degree in Social Work, Behavioral Health, or a related field.
• Experience working with at-risk or vulnerable populations.
• Knowledge of housing resources and services for individuals with serious mental illness.
• Understanding of Permanent Supportive Housing and evidence-based practices.
• Strong organizational, communication, and case management skills.
• Ability to handle crisis situations, including eviction prevention and homelessness intervention.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate and manage referrals from DBH, FBOP, Saint Elizabeths Hospital, and other federal and community organizations.
• Conduct initial screenings to determine client eligibility based on program and referral criteria.
• Collect, review, and maintain required documentation, including identification, clinical records, CRF packages, SPDAT assessments, and Community Partnership applications.
• Assist clients with placement into appropriate housing options, including SAFE housing environments and recovery-based housing programs (e.g., Oxford House).
• Conduct home visits and housing inspections to ensure compliance with health and safety standards.
• Develop and maintain relationships with landlords, housing authorities, and community partners.
• Coordinate care and services with external providers to support client stability.
• Provide referrals to additional support services, including behavioral health, legal, and temporary housing resources.
• Conduct ongoing monthly client visits to monitor housing stability and provide continued support.
• Stay informed on local housing regulations and advocate for client needs.
