Job summary

LHH in Costa Mesa, CA is seeking a compassionate Case Manager to support individuals experiencing homelessness through outreach, intake, resource navigation, and housing stabilization services throughout Central Orange County. This is a field-based and office-based role with a pay range of $23–$26 per hour.

You will conduct intakes, meet clients in community settings, connect them to housing resources and benefits, and develop service plans while maintaining documentation.

Qualifications

• Experience in case management, homeless services, housing navigation, social services, behavioral health, or a related field.
• Strong communication, engagement, and relationship-building skills.
• Ability to work independently in the field and manage multiple priorities.
• Knowledge of community resources and supportive services preferred.
• Strong organizational and documentation skills.
• Comfortable working with diverse and vulnerable populations.

Responsibilities

• Conduct client intakes, assessments, and enrollments.
• Meet with clients in community and public settings throughout Central Orange County.
• Build rapport and establish trusting relationships with individuals experiencing homelessness.
• Connect clients to housing resources, benefits, healthcare, employment services, and community programs.
• Assist clients with completing applications and required documentation.
• Support clients as they transition into housing and work toward stability.
• Develop individualized service plans and track client progress.
• Maintain timely and accurate case notes and program documentation.
• Collaborate with community partners and internal support teams.
• Empower clients to achieve their personal goals while encouraging accountability and self-sufficiency.
